How Do I Know if I Am Eligible?

Cape Fear HealthNet currently serves uninsured residents of  Brunswick, New Hanover, Columbus and Pender counties with incomes below certain guidelines.  For quick reference you can check the chart below for income limits.  Find the number of people in your household and then see if your income is equal to or below the limit shown.

Understanding whose income to count and what income to count can be confusing, so it’s best to check with a  Cape Fear HealthNet Enrollment and Eligibility Specialist about your possible eligibility.  Our Enrollment and Eligibility Specialists will make the application process as simple as possible.

Once completed, this form is sent to CFHN, and someone from CFHN will contact you to set up an appointment to speak to an Eligibility Specialist. The information you enter is completely confidential and there is no cost for this service. Please use this form only to request services for yourself (adults over the age of 18) or an adult for whom you have legal guardianship. Consent submitted through this form should be signed by the person who would be receiving services or by their legal guardian only.
